Workers Compensation for Patients

On your initial consultation we will need the following information:

​

• Employer name, phone number and address

• Claim number (if allocated)

• Date of injury (if applicable)

• Insurance company name (if allocated)

Please note that there is no legal or operational basis for employers or their representatives to be present during a medical consultation between an injured worker and a treating doctor. This applies even when treatment is recommended or facilitated by the employer.
